Use this form to request a cash withdrawal from your FCMM Retirement Plan account. To qualify for this cash withdrawal type, you must have incurred expenses associated with the birth or legal adoption of a child. The amount requested is limited to $5,000 per child born or legally adopted within the past 12 months. Qualified distributions are penalty-free and [optionally] may be repaid to the Plan. Use this form to request a cash withdrawal from your FCMM Retirement Plan account. To qualify for this cash withdrawal type, you must be a victim of domestic abuse that occurred within the past 12 months. The amount requested is limited to the lesser of $10,300 in 2025 (indexed for inflation, thereafter) or one-half of your account balance.
