Use this form to request a cash withdrawal from your FCMM Retirement Plan account. To qualify for this cash withdrawal type, you must be a victim of domestic abuse that occurred within the past 12 months. The amount requested is limited to the lesser of $10,300 in 2025 (indexed for inflation, thereafter) or one-half of your account balance. This is a penalty-free distribution that [optionally] may be repaid to the Plan. To request this form, please call FCMM Client Services at (800) 995-5357.
