file 09 Cash Withdrawal (Distribution) Request - One-Time

Use this form to request a cash withdrawal from your FCMM Retirement Plan account. To qualify for this cash withdrawal type, you must meet one of the following eligibility criteria: Age 59 ½; Severance of Employment; Disability; Alternate Payee in a QDRO; Beneficiary of Deceased Participant; or only requesting funds previously rolled into the FCMM Retirement Plan from another financial institution or the former pension plan. To request this form, please call FCMM Client Services at (800) 995-5357.
