file 11 Hardship Withdrawal Certification

Use this form to request a cash withdrawal from your FCMM Retirement Plan account. This is an in-service distribution limited to those with an immediate and heavy financial need who do not meet the eligibility criteria of other withdrawal options in our Plan. The IRS restricts the availability of funds for hardship withdrawal to the following reasons*: Medical Expenses; Costs directly related to purchase of primary residence; Payment of post-secondary tuition and related fees; Payments necessary to prevent eviction/foreclosure; Burial or funeral expenses; Expenses for repair of damage to principal residence; and Expenses or losses incurred as a result of a FEMA disaster. To request this form please call FCMM Client Services at (800) 995-5357.  *Additional requirements noted on the form.
